LONGEVITY DIRECTED THERAPIES AND RESEARCH

Individuals will be given the option to initiate therapies intended to slow down the aging process.  Examples include metformin, rapamycin, exosomes, NAD+, and etc. Those utilizing longevity directed therapies must visit with the physician at recommended intervals and comply with all recommended monitoring.

HOW TO JOIN
Schedule a "New Patient Consultation" for $200. After joining, your monthly payments will start immediately. If you have not joined the clinic since your initial consultation and 12 months or more has passed, please schedule a "Longevity and Performance Consultation." Wonder Medicine reserves the right to deny membership to any patient that the clinic does not feel they can adequately care for.

LONGEVITY + PERFORMANCE PROGRAM MEMBERSHIP AGREEMENT

• Patient agrees to pay for the Longevity and Performance Program (hereby: “program”) for 1 full year as one lump sum payment or divided into 12 equal monthly payments. Patients must enroll into the program for no shorter than 1 full year. Patients will automatically be enrolled into the Longevity and Performance Program in subsequent years unless a membership Termination Form has been received.

• Patients may quit the Longevity and Performance Program at any time by filling out a Termination Form (available at wondermedicine.com) and emailing the form to info@wondermedicine.com. Wonder Medicine will continue to collect payment until a completed termination form has been received, even if patient has verbally told Wonder Medicine that they plan to quit now or in the future. Any payments already received and any time already paid for are non-refundable and non-reusable after a dated Termination Form has been received. IF a patient is on a monthly payment plan and submits a Termination Form, prior to completing all 12 payments, then the patient is still responsible for completing all 12 payments in order to pay for 1 full year.

• Patient must complete all Longevity and Performance Program visits, questionnaires, screenings, and recommendations in order to continue to be enrolled in the program. Patient may choose not to participate in physical fitness testing. However, all other testing is required and must performed in the order outlined and mandated by Wonder Medicine.

• The Longevity and Performance Program accepts patients of all ages. However, patients must be able to participate willingly in the program. Generally, the program is recommended for those age 14 and up.

• Health Metrics obtained in the Longevity and Performance program are for screening purposes only and should be followed up with additional or confirmatory testing to be provided by patients primary care doctor or other medical providers outside of Wonder Medicine.

• Patient’s enrolled in the program agree to make improvements to patients life and health including improved nutrition, regular exercise, focus on sleep, and self care. Patients that are not willing to make improvements to their health may be asked to leave the program.

• Wonder Medicine reserves the right to deny any patient membership for any reason, at Wonder Medicine’s sole discretion.  

• The patient agrees to inform Wonder Medicine immediately if the clinic or its providers have caused them harm or dissatisfaction in any way. Wonder Medicine prides itself on constant improvement. If patient is unhappy with the membership, please let Wonder Medicine know why so that improvements can be made in the future.

HOW TO CANCEL

We require written notice, via email or direct mail that you would like to cancel. Please include the exact date you would like your membership to end. Please note that all Longevity and Performance Memberships are only able to be canceled at 1 year intervals. We will then email you a "Termination Form" to be completed and signed by the patient and emailed back to us. Once we receive a completed and signed "Termination Form," recurring payments will be stopped for that specific date provided. Any payments already received are non-refundable.

Wonder Medicine does not accept any form of insurance and does not provide any assistance for patients attempting to bill insurance directly. Wonder Medicine is not an emergency department, urgent care, or primary care clinic. Patients in need of emergency services or primary care should dial 911 or establish with a primary care clinic. Wonder Medicine is able to see patients covered by Medicare and Medicaid. However Wonder Medicine cannot provide or bill for services Medicare/Medicaid will provide. As such, medication management for mental health and chronic disease is not available for those also covered by Medicare/Medicaid.
